IVF – Egg donor Cycle



Planning the cycle:


An Egg Donor IVF cycle consist of the following protocol:


1. Selection and screening of Egg Donor.
2. Synchronised Donor stimulation and Intended mother’s endometrial preparation

 a. Donor Stimulations, Follicular monitoring of Egg donor and

 b. Endometrium Preparation of Intended mother.

3. Egg Pick Up & IVF or Cryopreservation of eggs/ embryos.

4. Embryo Transfer to Intended Mother or freezing of embryos. Hormone Assays.



Egg Donor IVF Treatment Plan:


1. Donor Selection and Screening:


We recruit egg donors through agencies or by placing advertisements. We screenthe donors in compliance to the guidelines provided by the Indian Council of Medical Research. We carry out the following Investigations:


  • HBsAg – Hepatitis B
  • VDRL - Syphilis
  • HCV – Hepatitis C
  • HIV I & II
  • TSH
  • PROLACTIN
  • BLOOD GROUP & RH FACTOR
  • HB
  • BLOOD SUGAR
  • CHEST X-RAY
  • THALASEMIA PROFILE (HB HPLC)

  • Apart from this, the Donors are also counselled by a Psychological counsellor.


    Matching of Donors:

    Intended Mother needs to provide us her picture so that we may begin our screening.


    We try to match the donor’s complexion, iris colour and blood group but it is notalways possible to match all the criteria. We generally do not charge extra for Indian donors with wheatish complexion. But if we have to screen for fair donors we charge extra. If further iris match is required additional charges will be applicable.


    As per Indian norms (ICMR guidelines) the Egg donor information is confidential. Wetherefore cannot provide any picture. We do however provide donor profile on paper on request. The donor profile will include Physical profile, Test Results, Basic Background Information like age, marital status, no of children.


    Once we have screened a suitable donor we contact the Intended parents.


    Special Donor Screening Services:


    We are associated with a number of Specialised International Egg donor agencies that can provide specialised donor screening services like matching race and ethnicity (Caucasian/ mongoloid), matching iris colour and complexion.


    These International agencies include as part of their services all or some of thebelow services:

    • 1. Donor Investigations.
    • 2. Donor stimulation till a specific day.
    • 3. Stimulation medicines.
    • 4. Follicular monitoring & hormone assays.
    • 5. Donor and chaperone’s Visa Processing, Booking Tickets, to and fro Travel costs.
    • 6. Donor & Chaperone’s accommodation, food, and local conveyance on arrival.


    When do we start screening for Egg Donors :


    The Egg donor screening is initiated once the Intended parents have booked for thecycle. Local patients can deposit a semen sample at this point which is cryo-preserved for one month at no additional costs. This acts as a back-up sample for IVF.


    Once the donor has been successfully screened we inform the Intended parents. Ifthe husband’s semen was already deposited for cryopreservation at the time of booking, we immediately start the donor stimulations.


    2. Donor stimulation and Intended mother’s endometrial preparation


    Donor Stimulation commences with ovarian Induction of Egg donor to achieve super ovulation and retrieve multiple eggs by egg pick up procedure.


    Endometrium Preparation involves administration of oestrogen hormone (EstradiolValidate 2mg twice/ trice daily) to the Intended mother along with folic acid supplements and Ecosprine till the desired endometrium thickness is achieved (usually takes 12-14 days) . Estragel (2.5 gms) needs to be applied on abdominal skin twice or trice daily for the duration. Simultaneously Endometrium Thickness study is done which involves regular measurement the Endometrium Thickness using Ultrasonography. It takes 10 – 12 days to usually attain the proper thickness which should ideally be at least 7mm and above.


    Synchronisation of Egg Donor and Intended Mother’s cycles.


    The donor stimulation starts with hormonal stimulation from Day 1 of Donor’smenstrual cycle. This is followed by follicular monitoring to study the growth and number of follicles formed and schedule the date of Pick up. Pick Up unusually takes place within 12-14 days of start of stimulation. IVF is done and embryos are formed within 24 hours of Pick up. The embryos are grown for 3-5 days (blastocyst stage) and then transferred to Intended mother. This entire process takes 15-20 days.


    The Intended mother must be therefore be ready to receive the embryos by the timethey are grown till the blastocyst stage. Therefore the endometrium preparation of the intended mother must also be started 20 days in advance so that she achieves the desired endometrium thickness of minimum 7mm by the time the embryos are ready for transfer.


    Local patients can visit the centre for USG scans and investigations during the endometrium preparation phase. International patients can coordinate over phone/ email and start their preparation in their respective countries. Our consultants will advice you on alternative brands of the medicines for Endometrium preparation that is available to you locally.


    3. Ovum Pick Up & IVF or Egg / Embryo cryopreservation


    Fresh transfer of Embryos to Intended mother can only be possible if the Donor Cycle and Intended mother are synchronised and the IVF with husband semen could be successfully performed on time. That means two factors determine the success of synchronised egg donor– fresh transfer cycles.

    1. Adequate good quality husband’s semen sample was available at the time of Pick Up.

    2. Endometrium of Intended mother is ready by the time embryos are in blastocyst stage.


    What happens if husband’s semen sample was not available?


    Donor stimulations and pick up takes 12-14 days. IVF is done within 24 hours of pickup. It is our standard procedure to take a husband’s semen sample one month in advance and cryopreserve it as a back-up sample for this purpose. If you wish to carry out IVF with fresh semen, then the Intended father needs to visit the centre on the date of Egg –Pick up and deposit fresh semen. If the semen sample is not adequate then fertilization is carried out with back up sample (previously cryo-preserved semen). If the fresh semen sample was not adequate and there is no back up sample, the eggs are cryo-preserved at extra costs. In such a case, the IVF is scheduled at a later date whenever the intended father can provide a fresh and adequate semen sample. Also the Endometrium Preparation cycle of the Intended mother is aborted as there are no embryos formed.


    What happens if Intended mother is not ready on time?


    If the endometrium preparation of Intended mother could not be synchronised with egg donor IVF cycle due to any reason, then either the eggs/ embryos can be cryopreserved. The extra cost of cryopreservation of eggs / embryos for 6 months will have to be borne by the patient.


    If all the conditions are met then Embryo Transfer of fresh embryos is scheduled.The Intended mother whose Endometrium preparation was started in sync with Donor stimulations has to visits the centre on the date specified for Embryo Transfer


    Planning the cycle for International Patients:


    International patients / out station patients have the option of undergoingEndometrium preparation locally (in their respective country) and come down to Kolkata centre on the day of egg pick-up. The donor stimulation takes approximately 12-14 days and the pickup is timed with your arrival as the fertilization will be done with your husband’s sperm. If the semen sample was deposited in centre earlier or in case donor semen is used, then intended mother can come on the day of embryo transfer to the centre.


    International patients / out station patients have the option of undergoingEndometrium preparation locally (in their respective country) and come down to Kolkata centre on the day of egg pick-up.
